eForm Record Page

The eForm Record page contains all information about an eForm and appears when you add a new eForm or edit an existing one. Your IQ user ID must have access to Security Lock #235 in order to create and modify your own eForms and Lock #234 in order to edit eForms created by other users. The title of the page shows the eForm ID, the eForm name and status. The record page contains panels that let you enter information about an eForm, define the content and appearance of the eForm, and preview the eForm. To access one of the following panels, click the appropriate tab: General, Names and Addresses, Fields, Cosmetics, eFormat, Review Form and Change Log.

General Panel

The General panel contains fields for entry of the eForm name and description along with the status, owner, delivery method and record creation. Information about the eForm usage and fields is also displayed. Note: If the status is Published, access to fields on this panel is restricted so that the information cannot be changed.

Fields on the General panel include: ID, Created, Revised, Name, Description, Status, Owner, Delivery, Create, Usage and Fields. Information is entered automatically in the ID, Created and Revised date fields by IQ and cannot be changed.

Name

Type a name, up to 50 characters, for this eForm. This name is displayed in the title of the record page as well as the Search Results page. If you do not enter a name, IQ will assign the name "Form ID#" in which ID# is the ID number assigned by IQ when the eForm is created.

Description

This optional field lets you enter information, up to 4,000 characters, about the eForm you’re creating. Information entered into this field is displayed in the Description column on the Search Results page. This is a text-entry field, which means you can type and delete text in this field just as in a word processor. You can also spell-check text entered into this field by clicking the spell check icon next to the field.

Status                                                                      

This field defaults to Draft when you create a new eForm. A status of Draft allows you to make changes to the eForm, however, you must change the status to Publish before the eForm can be sent. Note: Your IQ user ID must be granted access to Security Lock #233 in order to change the status to Publish.

Owner

The Owner field is automatically populated with the name of the IQ user who created the eForm, however, it can be reassigned to another IQ user if needed.

Delivery

This field allows you to control the method of delivery for the eForm. Click in this field and select one of the following options:

E-mailed to people already in the database

This option is used for responses to emails sent out from IQ.  IQ will track the name and address information from the People record of the correspondent.

Web browser – Prompts for Name and Address Information

This option allows the person who is completing the eForm to enter his or her name and address information.  If you select this option, IQ displays a second field with the options to allow or not allow additional names and addresses to be included in the eForm.

Create

The Create field allows you to choose whether the eForm will be used to create new Mail records via IMA Correspondence Rules, or whether the eForm will be used to create new Workflow records in IQ using a selected template.

Usage

Once this eForm has been used, this section is updated with the usage counts for the number of form letters, pending and completed mail records that use this eForm. You can click the view link to go to a listing of the form letters or mail records that contain this eForm.

Fields

This section of the General panel contains a list of the questions that have been defined or fields that have been selected on the Fields panel.

Names and Addresses Panel

If you selected the Web browser – Prompts for Name and Address Information option in the Delivery field on the General panel, you can select the fields you would like to appear on the web form and specify which fields will be required, which will appear but not be required, and which will not appear.

If you selected a workflow template that will allow the user to enter additional People records, IQ will display an additional section for you to select the characteristics of the People record fields for non-primary people.

Fields Panel

The Fields panel allows you to select fields, enter questions and determine the layout of the information you are requesting in the e-mail or web site. Select the Add Form Fields action to create the first row and select the field, type, required, and display value. You can continue to use the Add Form Fields button to enter an additional row at the bottom of the list, or you can use the Ins button to add an additional field or section break in between existing rows. The Del button allows you to delete a row. The up and down arrow buttons allow you to move the current row up or down one position.

Field

The options that appear in this drop-down list are based on whether or not the eForm is creating a mail or workflow record. Select Section Break to create a section heading and/or provide instructions for data entry. Select Primary Person if the eForm will be delivered via a web browser and does not create a workflow. If the eForm will create a workflow then the drop-down list will contain fields that are available in the selected workflow template in the Create field on the General panel. Fields are also available for prompting for an e-mail subject, message text, and codes (affiliation, issue and workflow).

Type

The Type field allows you to control the display of the Answer List field values as either single values (text box, drop-down list or radio button list) or multiple values (text box or checkboxes). In addition, you can identify the answer list values as a date, number or hidden field.

Required

This allows you to identify which fields the recipient is required to complete before he or she can submit the eForm. Click in this field and select Yes from the drop-down list to identify a field as required.

Height

This field is available when you select Multi Line Text Box in the Type field and allows you to control the number of lines for the text box.

Columns

This field is available whenever you select Single Line Text Box, Multi Line Text Box, Radio Button List, or Checkbox List in the Type field. For single line or multi line text box, this field allows you to specify the width of the text box.  For radio button or checkbox lists, this field allows you to specify the number of columns used to display the Answer List values.

Max

This field is available when you select Single Line Text Box or Multi Line Text Box in the Type field.

Display Value

If you selected Section Break in the Field drop-down list, then enter the section heading or instructions in this field.  For all other field types enter a label to describe the field input. For example, if the selected field is a workflow data field called SS#, then the display value could be "Social Security Number".

Answer List

This field is available whenever you select Dropdown List, Radio Button List, or Checkbox List in the Type field. Type each answer on a separate line in the format Value or Value=Display Value. If you select Affiliation Code, Issue Code or Workflow Code in the Field field, IQ will display a list of existing affiliation, issue or workflow codes for your IQ database. To select a code and its translation, click the code link or you can use the Filter Values field to search for codes that contain a word or partial word in the ID or Translation. So, for example, if you search for "national", IQ will return codes that contain "national" or "international" in the ID or translation. Click the Refresh button to clear the filtered list and restore the original list of codes.

You can include a mix of Affiliation Codes and non-Affiliation Codes in the Answer List. In addition, you can change the display value to something that is more descriptive than the translation. For example, if your IQ database contains the Affiliation Code COC with a translation of Chamber of Commerce, it would appear in this field as COC=Chamber of Commerce and "Chamber of Commerce" would be displayed in the eForm. If, however, you would like "Chamber of Commerce Member" to appear in the eForm, you would simply add the word "Member" to the end of "COC=Chamber of Commerce". Note: IQ uses the Affiliation Code ID (i.e. COC) to update the recipient's People record in your IQ database.

To remove an affiliation code, include the word REMOVE in front of the affiliation code in the answer list. For example, the answer list for an affiliation code used for subscribe and unsubscribe might be:

    1. EMAIL_OPTION = Yes, I would like to subscribe to your weekly eNewsletter

    2. REMOVE EMAIL.OPTIN = Please remove me from your list

Cosmetics Panel

The Cosmetics panel allows you to select the color, font type and size for the display of the question, answers, radio/checkbox lists and section breaks in the eForm. In addition, you can select a separator, the background color and body style settings. When you click in the Font or Color fields, IQ displays a scrolling list of available fonts or colors. You can copy and paste a font or color from the list into the field or manually type it. The list of colors contains the color name along with its HTML color code. You can enter either in the Color, Form Color or Body Style Settings fields.

Click in the Link Text field and enter the text that you would like IQ to use for the link that opens the eForm in a new browser window. When the recipient submits the eForm, IQ will display a pop-up message with the text that appears in the Thank You Message field. The button text for the "Submit" button can be changed in the Button Text field. You can then redirect the recipient to another web site, by typing its URL in the Redirect URL field. Click the Test button to review the thank you message or redirect URL. If you have entered a valid redirect URL, IQ will open it in a new browser window.

eFormat

The eFormat is the frame within which your eForm is presented, whether you are delivering the eForm via e-mail or a web browser.  Use graphics, text or any other objects you wish to create a sophisticated presentation.

Review Form Panel

The Review Form panel allows you to preview how the eForm will appear based on the fields, cosmetics and eFormat that have been selected.

Change Log Panel

The Change Log panel contains a history of changes made for this eForm record, including the user who performed the change, the date and time the change was made and a description of the change.
